To help lower the cost of health care expenses for you and your family as well as the Fund, the Local 825 Welfare Fund offers several networks which provide access to medical providers, hospitals, dentists and pharmacies at negotiated discount rates.<\/p>\n

When you or a family member need care, you have the freedom to choose a provider. Depending on where you decide to receive care, your share of costs varies.<\/p>\n

Additionally, providers may not keep their information updated OR all providers within a listed group may not participate. Check with a provider’s office at the time you call to schedule an appointment to confirm a provider’s participation status.<\/p>\n

Please note that selecting a participating provider and\/or hospital does not guarantee that all service providers at the hospital are in network. Charges of anesthesiologists, pathologists and radiologists are commonly not included in hospital contracts. Claims for these out-of-network providers will be paid in the same manner as other out-of-network claims<\/p>\n\t\t
